Two clarifications matter more than the headline.
Removing a boxed warning does not delete a risk. Venous thromboembolism, ischemic stroke, and gallbladder disease remain in the Warnings and Precautions sections. Clinicians and patient advocacy groups have cautioned against a pendulum swing in the opposite direction, particularly for breast cancer survivors.
And the labeling change did not come from new trial data. It came from re-reading existing data by age group.

Genitourinary symptoms sit in their own category. Vaginal dryness, painful intercourse, and recurrent urinary symptoms respond to low-dose local vaginal estrogen, which produces minimal systemic absorption. Its safety profile differs meaningfully from systemic therapy, and it remains an option for many women who are not candidates for systemic treatment. That decision still belongs with your clinician, particularly after breast cancer.
Delivery Routes Are Not Interchangeable
Route changes risk, not just convenience. Oral estrogen passes through the liver first, which raises clotting factors, triglycerides, and gallstone risk. Transdermal delivery bypasses that first pass.
